To allow us to promptly review the referral and offer the patient a convenient appointment time, please ensure the patient's name, date of birth and contact phone numbers are clearly visible. Referrals can be FAXED to any of the following numbers:

*Note: Private hospital referrals CAN be faxed to the out-patient number, and vice-versa. We provide multiple faxing options for convenience for referrers who are located a distance from our administration centre. Similarly, most referrals are interchangeable - we will always attempt to offer the patient the most appropriate study location.

Online referral (Medical Objects)

SNORE Australia now accepts electronic referrals through Medical Objects (to either Dr John Corbett or Dr Sean Tolhurst, at any of the practice locations listed in Medical Objects). Once the referral has been reviewed by one of the Credentialled Sleep Physicians, our staff will contact the patient to arrange a suitable appointment.

 

How to Book a Sleep Study

As SNORE Australia is a medical provider and you are under the care of our Credentialed Sleep Physicians, a referral from your doctor is required for an overnight or daytime sleep study to be booked.

Once your referral has been received, our staff will contact you to offer a suitable appointment and ensure the correct investigations for you are arranged.
